Our pledge

Information about you and your health care is personal. We create a record of the care and services you receive, and we need it to provide quality care and meet legal requirements. We are required by law to keep your protected health information (PHI) private, to give you this Notice of our legal duties and privacy practices, to follow the terms of the Notice currently in effect, and to tell you if there is a breach of your unsecured PHI. You may ask for a paper or emailed copy at any time. We may change this Notice, and the new terms will apply to all PHI we keep. The current version is always available on our website and in our office.

  • to remind you of appointments and to tell you about services we offer.

When we use or share PHI, we make reasonable efforts to limit it to the minimum necessary, except where the law says otherwise.
